The DisAbled
Woman's Network (DAWN) Ontario is looking for individuals and
organizations who understand that by including people with disabilities
in all life activities we are all enriched. Help us look for those
individuals and groups in the province of Ontario who understand
the value and beauty of participating together.
What is inclusion?
Inclusion is
people with and without disabilities participating together in all
life activities - at school, at work, at home, at worship, in the
community.
How to make
a nomination:
Get
the Nomination form as a WORD document
Get the Nomination form as
an RTF (rich text file) document
Complete the
Nomination form and submit via regular mail before April
30th, 2001 to…

Prior to giving
the award, DAWN Ontario may ask to make a site visit or to interview
the nominee.
Please read
the entire Nomination form before answering any questions. Make
your nomination no later than mid April to ensure it is received
by April 30th, 2001.
